Consent Form for the Disclosure of Personal Data

I, as a service user (“Customer” or “User”) of EV Serve under the EV World (Thailand) Group (“Company”), hereby consent to the Company collecting, using, or disclosing my personal data held by the Company under the following terms and conditions:

Terms and Conditions — Types of Personal Data

General InformationGeneral Information Means information relating to an individual that enables the identification of that person, whether directly or indirectly, but does not include data of deceased persons specifically.

Sensitive and Financial InformationSensitive and Financial Information Means any information or materials expressed in the form of documents, reports, photographs, video or audio recordings, electronic instruments, or any other method that makes recorded information visible, relating to financial information, payment slips, or any data that can identify an individual.

Purposes of Collecting, Using, or Disclosing Personal Data

To provide services that meet the needs of customers, the Company will use the customer’s personal data so that the customer can receive products and/or services according to the customer’s intended purposes, under contract or upon request, in the following cases:

For consideration, approval, and/or provision of various services For any operations related to providing products and/or services, such as processing, contacting, notifying, transferring rights and/or duties, customer coordination. If customers refuse to provide information, the Company may be unable to offer products and/or provide services.

To comply with legal obligations For compliance with applicable laws such as securities and exchange laws, tax laws, anti-money laundering laws, computer laws, and bankruptcy laws. The Company may collect, use, and/or disclose personal data such as information about directors, authorized persons, representatives of corporate clients, or individuals under court protection orders.

Risk management, audit, and internal operations Including prevention of unlawful activities such as fraud, cyber threats, money laundering, and other legal violations.

Security of personal data Such as anonymizing personal data so that individuals can no longer be identified (Anonymous Data).

Business Purposes

The Company collects personal data for the purposes of statistical analysis or corporate communications, such as photo or audio recordings during meetings, training sessions, recreational events, or booth activities. If customers refuse to provide such data, the Company may be unable to provide services efficiently.

To ensure customers receive full benefits from products and/or services to which they have consented, such as receiving offers, special privileges, recommendations, news updates, or rights to participate in special activities. Refusing to provide data may prevent the Company from delivering services effectively.

Data Providers

Social media platforms and advertising service providers, for the purpose of updating customer data and improving product and service quality and efficiency.

Retention Period of Personal Data

The Company will retain personal data for as long as necessary to fulfill the purposes stated in this Privacy Policy, unless laws require or permit a longer retention period. This may be approximately 3–5 years or longer as necessary, such as for legal prescription periods, litigation purposes, or regulatory audits.

Disclosure of Personal Data

The Company will not disclose personal data to any person without first obtaining consent from the customer. For any disclosure, the Company will require the recipient to keep the data confidential and not use it for any purposes beyond those defined by the Company.

If the customer believes that a recipient of their personal data has used the data beyond the Company’s specified purposes, the customer may notify the Company using the contact details stated in this Privacy Policy for further action.

Rights of the Data Subject

Editing or Deleting Personal Data Customers may edit or delete their personal data at any time. Customers may request a “Personal Data Request Form” to complete and submit to the Company via EV Serve Contact Center: contact@evservethailand.com or Tel: 098 889 9883. Please note that deletion of data may prevent the user from receiving services from the Company or may affect service efficiency.
